Forms: Membership Application - Instructions
This is your opportunity to become a member of a full-service financial institution and enjoy all the benefits of credit union membership. Please be sure you have read all of MemberSource's membership eligibility requirements before submitting your application.
Instructions for Application:
- You may print the Adobe Acrobat membership application to hand write your information. Or you may use our Word form to type your information. Either way, you will need to complete all necessary information, print the form on white paper and sign.
- Include a check or money order for at least the minimum initial required deposit(s):
- Savings (required account) - Minimum $5 to open
- Christmas Club - Minimum $25 to open
- Youth Account - Minimum $5 to open
- Simply Free Checking - Minimum $25 to open
- Member's Advantage Checking - Minimum $500 to open; $500 minimum balance required to avoid a $6 monthly service charge.
- Market Reward Checking - Minimum $5,000 to open
- Include a clear copy of all account holders driver's license or state issued ID.
- Include a copy of a recent paycheck stub.
- Mail your application, check or money order and all required information to:
